It seems as if your browser can't find or doesn't have access to your Camera and your Microphone. Please make sure you have those enabled. If you're on MAC, please make sure Safari or the browser you're using has access to your Camera and Microphone:

 

System Preferences --> Security & Privacy --> Camera (turn on for your browser) --> Microphone (turn on for your browser) and you'll be good to go!
